Each card says when to use it, what to decide, the rules for deciding, which template it fills and an optional prompt. The prompts are optional; every card works on paper.

A complete worked example
The most useful evidence came from explanations, not from right answers. The "unverified versus false" misconception needed two follow-ups in new contexts before most learners held on to it. The assessment was reported by criterion, and the weakest criterion became the next teaching focus.
The example is fictional: it demonstrates the method and is not evidence of results for any particular person.
